Approximately 12.6 million WIN FOR LIFE!® tickets are initially planned in this game. The New Jersey Lottery reserves the right to subsequently increase this quantity of tickets. Should additional tickets be introduced, prize levels and frequency of winning will be consistent with the initial quantity of tickets. In the WIN FOR LIFE!® Scratch-Offs Game, New Jersey allocates approximately 66% of the gross receipts, net of free tickets, to prizes. On the average, better than 1 ticket in 5 wins a prize. Odds and number of winners may vary based on sales, distribution and claims.

CLAIMING OF "$5,000 A MONTH FOR LIFE!" PRIZES

Following the validation of the ticket, the "$5,000 a Month for Life!" prize (guaranteed minimum prize: $1,500,000, payable in 25 annual installments) will be issued annually in advance starting on or about the anniversary date of the claim and will continue for the life of the winner. The "$5,000 a Month for Life!" minimum prize of $1,500,000 is guaranteed to the winner or his or her estate. At the time the prize is claimed, the claimant shall be entitled to change the election to a cash option, where the winner shall be entitled to the gross cash value of $875,200, provided, however, that no change to the election shall be permitted after payment of a cash prize, or after payment of the first installment of an annuity prize, or subsequent to the sixtieth day following the date at which the prize was claimed.

Winners of "Win for Life!® Win Up to $5,000 a Month for Life!" prize must provide a valid birth certificate for purposes of determining the winner’s age. Neither the "$5,000 a Month for Life!" prize nor any benefits attached thereto will be paid to any person under the age of eighteen years. Rather, a winner of the "$5,000 a Month for Life!" prize who has not yet attained eighteen years of age shall only be entitled to either (1) a single gross lump sum prize payment of $875,200 or (2) a series of installment payments over a term of 25 years totaling $1,500,000. Such prize, either lump sum or 25 annual installments, will be payable to any bank to the credit of an adult member of the minor's family or a guardian of the minor, as custodian for such minor, under the New Jersey Uniform Transfers to Minors Act in accordance with N.J.S.A. 5:9-20. This provision does not permit the purchase of tickets by minors prohibited by N.J.S.A. 5:9-15. Amounts of lump sum prize payments may vary depending on prevailing interest rates on the date a prize claim is approved by the Lottery, which interest rate is used to compute the present value of the prize payment.

Any ticket claimed by a family, group or corporate name will be given the opportunity to file an amended claim in one individual name whose life term would be used for purposes of paying the annuity prize (the "identified life term"). This would be accomplished by each member of the family or group granting power of attorney to the individual of his or her choosing. The principals of a corporation would also be given the opportunity to choose an individual in a similar manner. Winners of the "$5,000 a Month for Life!" prize must provide a valid birth certificate for purposes of determining the winner's age. Once the claim is filed, the identified life term may not be subsequently changed. Any assignment of a "$5,000 a Month for Life!" prize shall be only for the portion of the guaranteed minimum prize remaining after subtracting the number of annual installments during which the "$5,000 a Month for Life!" prize has been paid from twenty-five.
